FOR RECEIPT OF GOOD AND VALUABLE CONSIDERATION, all ownership rights and interest of [Name of Assignor], hereinafter referred to as Assignor, in the following accounts receivables: is hereby conveyed and assigned to [Name of Assignee], hereinafter referred to as "Assignee".
This transfer is made pursuant to the terms of a contract between the Assignor and the Assignee dated [Date of assignment].
Should any of the accounts transferred be uncollectible in part or in whole, the account of the Assignor shall be adjusted in accordance with the balance of the uncollectible amount.
